Registration Form
Please download and complete the following forms below. Bring them with you on your first visit or email beforehand to marie.bobbi@gmail.com.

Space is limited each day. It is on first come, first serve bases.

You will need to fill out completely for EACH child.
You are required to list at least 2 emergency contacts (not including guardians).
Please print, sign and send in or bring with you before or on your first visit.
Information
Hours: Monday – Friday (closed on most holidays)
Times: 7:30am – 5:00pm (rates double if child is not picked up by 5:00pm)
We will be closed for personal vacations and will provide notice.
Available is always subject to change. Please contact us prior to drop off.
Rates
1st Child: $8.00 per hour
Each Additional Sibling: $3.00 per hour
Rates are pro-rated to the half-hour with a 1 hour minimum charge.
Multiple child rates apply to siblings only.
We accept All Major Credit Cards(Visa/Mastercard/AmEx/Discover) and Cash. No personal checks will be accepted.
Payment is due when services are rendered.
